Position Summary:

Join NeighborHealth's Neighborhood PACE program where the PACE Social Worker role offers exciting new challenges daily. As part of a dynamic Interdisciplinary Team (IDT) in an innovative model of care, you will develop care plans for participants wishing to live safely in their communities. You will work with participants at the PACE Center and in their homes, addressing issues such as high-risk mental health, end-of-life care, medical decision-making, caregiver support, and more. Responsibilities include conducting psychosocial assessments, formulating treatment plans, providing counseling, and opportunities for leading groups and coordinating care with team members to help participants achieve their goals. Must have an MSW and be passionate about serving adults aged 55+. Experience working with older adults is preferred. Must possess or be working toward licensure as a Licensed Certified Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) in MA. Bilingual (Spanish speaking) required.

Essential Duties and Responsibilities:

  • Responsible for intake, including administering psychosocial assessments and identifying factors affecting the participant’s functioning.
  • Completes initial assessments and contributes to the care plan by recognizing how PACE can support the participant’s well-being and community engagement.
  • Provides caregiver support and leads family meetings.
  • Participates actively as part of the IDT, sharing assessments and writing treatment interventions in the care plan, collaborating with participants on their goals.
  • Develops and leads activities demonstrating clinical expertise, such as group counseling, resource management, psychotherapy, end-of-life care, supervising interns, or presenting at ethics meetings. Continues professional development through education and applying new knowledge.
  • Coordinates admissions and discharges, providing care coordination for participants needing psychiatric hospitalization.
