Job description

Job Title: Outreach Recovery Practitioner
Location:South East London
Rate:£22 - £23 P/H Ltd Umbrella (£20.33 PAYE Inclusive)

Responsibilities:

• Conduct outreach shifts in all weather conditions and at various times, including early mornings, nights, and weekends
• Engage with individuals experiencing rough sleeping and those involved in street-based activities
• Collaborate with partner agencies to help individuals access accommodation, health services, substance use support, and other essential services
• Accurately document interactions with individuals and professionals
• Work flexibly across different operational sites and be available during unsociable hours, weekends, and severe weather conditions
• Continuously improve personal performance, knowledge, and skills through training and development opportunities
