Job summary

Nottinghamshire Healthcare NHS Foundation Trust is seeking an Assertive Outreach Nurse to deliver flexible, person-centred support to individuals experiencing severe disadvantage across Nottinghamshire County. The role is community-based, delivering clinical outreach and substance misuse interventions in housing, health venues and outreach settings.

The post requires a first-level registered nurse with an active NMC PIN, the ability to work autonomously, and a commitment to trauma-informed,

Qualifications

First level registered nurse with current NMC PIN.
Experience in community outreach and multidisciplinary teams.
Knowledge of substance misuse treatment, harm reduction and health promotion.

Responsibilities

Deliver clinical outreach interventions to individuals experiencing severe disadvantage in community settings.
Undertake comprehensive physical assessments and support access to healthcare services.
Administer Buvidal injections in line with prescribing guidance and protocols.
Provide harm reduction advice, overdose prevention education and wound care support.
Coordinate referrals and ensure timely access to alcohol treatment and recovery services.
Train others in Naloxone use and promote access across the population.
