MUST COMPLETE ONLINE TRAINING AND PASS EXAM AT END OF TRAINING.Position Summary:The Navigator will ensure timely enrollment and re-enrollment of clients in the CHN uninsured program and those seeking services for the Federally-Facilitated Marketplace (FFM). The Navigator is a team member who assists with providing a coordinated system of health care to low-income uninsured residents and residents seeking health insurance from the federal marketplace. The individual in this position must possess the ability to interact well with supervisors, co-workers, medical professionals, enrollees and various stakeholders. They will be responsible for client program renewal, community education and outreach.
Essential Job Responsibilities:- Perform enrollment, re-enrollment and annual renewals for CHN clients.
- Perform the task of a certified ACA navigator to include expertise in eligibility, enrollment and program specifications for the community.
- Assist individuals with the enrollment process for CHN and ACA and ensure that all eligibility criteria are met. This includes taking referrals, completing enrollment forms, interviewing the client, verifying information, maintaining data and making changes to information as necessary.
- Conduct public education to raise awareness about the Federally-facilitated Marketplace (FFM) and the CHN uninsured program.
- Provide education and information in a culturally and linguistically appropriate manner.
- Ability to link, refer, and follow-up.
- Conduct eligibility reviews and establish continued eligibility of enrollees. The employee must determine how changes impact eligibility.
- Expected to pass all HHS Navigator approved examinations and receive Navigator certification and recertification from CMS.
- This individual must also possess the ability to recognize situations that should be referred to leadership.
- Notify enrollees of their need to complete the CHN Eligibility Review Form to verify continued program eligibility and prevent discharge from program.
- Maintain data and quality reports for the program.
- Cultivate and maintain collaborative relationships with member organizations, providers, partners, and other community-based organizations to engage their assistance in working with enrollees.
- Participate in training and annual certifications as necessary to successfully complete job responsibilities.
- Participate in CHN activities, staff and department meetings.
- Abide by all CHN, federal, state, and county policies, procedures and mandates.
- Perform any other related duties as assigned.
- Represent the agency in a positive manner at all times.
Essential Job Qualifications:- Associate degree or equivalent preferred.
- CMS Navigator Certification.
- At least one year of caseworker, case management or related experience preferred.
- Strong computer skills, including Microsoft Word, Excel, PowerPoint and Access database systems.
- Previous experience with database and website management, and social media sites.
- Strong oral and written communication skills.
- Experience working with healthcare programs/services, community organizations, social services and public resources.
- Strong organizational and time management skills.
- Bilingual preferred.
- Ability to communicate effectively with the public and applicants.
- Ability to maintain a positive demeanor.
- Flexible under pressure.
- Experience working with homeless and low-income preferred.
