Job summary

Bridgend County Borough Council is seeking an Outcomes Focused Assessor for the Commissioning for Complex Needs Team. The role centers on strengths-based, progression-focused social work to support adults with complex needs to achieve meaningful outcomes and independence.

About the Role

At the heart of this role is progression.

We believe support should create opportunities for people to progress, build on their strengths and achieve the outcomes that matter most to them.

Using strengths-based and outcome-focused social work practice, you will work alongside individuals, families, carers, providers and partner agencies to develop progression plans that promote independence, wellbeing, choice and control. Bringing professional curiosity, creativity and critical thinking to complex situations, you will help identify opportunities, overcome barriers and support people to achieve their goals and aspirations. Through assessments, reviews and collaborative working, you will help ensure people have the right accommodation, care and support to achieve the outcomes that matter most. You will work closely with providers and partners to ensure support remains focused on progression, enabling people to develop skills, build confidence and move closer to living the life they want to live.

You will also play an important role in supporting access to NHS Continuing Healthcare (CHC) and other funding arrangements, helping to ensure people receive the right support at the right time.

Alongside your direct work with individuals, you will contribute to developing solutions for people with complex needs and help inform future accommodation, care and support options, ensuring services continue to evolve to meet changing needs.
