Job description

Our Out of Hours Service is a vital part of Adults Specialist Services, providing urgent and long-term care. We seek Band 5 Community Nurses to enhance our North Somerset Team.

As the South West's largest community healthcare provider, our multidisciplinary teams—including nurses, therapists, clinicians, and support staff—help individuals stay safe and live independently following injury, illness, or hospital stays. We adopt a holistic, person-centered approach involving individuals in their care to promote thriving in their homes and communities.

Work Pattern

  • 36 hours per week, flexible
  • 3 nights a week, 19:30-08:00 with a 1-hour break

Main Responsibilities

  • Assess, plan, implement, and evaluate patient-centered interventions using specialist knowledge and training
  • Perform clinical skills such as catheterization, pain management, wound and end-of-life care, and IV medication
  • Promote shared decision-making and empower self-management of long-term conditions
  • Supervise colleagues and contribute to service development

Person Specification

Qualifications (Essential): BSc or equivalent in adult nursing, current NMC registration, evidence of CPD and lifelong learning

Skills, Experience, and Knowledge (Essential): Effective communication, clinical reasoning, autonomous working, problem-solving, time management, reflection, IT skills

Desirable: Knowledge of Evidence-Based Practice, health and safety policies, experience with multidisciplinary teams, community work
