A leading healthcare provider in the UK is seeking an Optimising Care Facilitator to support diabetes management through tailored education and behaviour change interventions. Candidates should possess NVQ Level 4 in Health and Social Care and demonstrate excellent communication skills to empower patients effectively. This role is ideal for those passionate about significantly impacting patient outcomes in diabetes care.

We are excited to offer an opportunity for an Optimising Care Facilitator worker to join our dynamic and supportive team at CHCP.
In this role, you will play a pivotal part in supporting individuals with diabetes to achieve better health outcomes through tailored education, guidance, and behaviour change interventions.
Additionally, this post requires that you have the ability to travel between locations to carry out your work.
Base will be confirmed once in post.
