An exciting opportunity has arisen for a band 4 Oncology SACT Care Coordinator to support the existing Oncology SACT Clinical Nurse Specialist Team with the delivery of cancer services and pathways at HUTH within the Queens Centre, Castle Hill Hospital. This is a dynamic team that has been recently been nominated at the HYCCN awards for going the extra mile, we are looking for an individual that is keen to be part of this ever evolving and growing service, that will continue to go the extra mile for their patients!!

Main duties of the job

The post holder will Work under the supervision of the Systemic Anti Cancer Treatment (SACT) Nurse Specialists (CNS) team in providing patient care co-ordination for patients undergoing SACT treatment. The role will assist the team in service development and support the team in implementing new initiatives and improving existing services such as nurse led services.

Will work in a close conjunction with the SACT CNS, they will help provide co-ordination of high quality patient care services involving ongoing telephonic and face to face assessment and proactive identification of needs utilising basic clinical knowledge, approved tools and procedures

Will act as a single point of contact for patient, carers, agencies and other Health Care Professionals (HCP). Triaging enquiries to the appropriate team or department within the hospital or community setting

Collaborate and communicate with SACT CNS team and wider health and social care team to ensure signposting/referral to the appropriate services to maximise health and quality of life for the patient, improving the quality and efficiency of health care services delivery

As a SACT care pathway coordinator, the post holder works with the SACT CNS team providing patient co-ordination for patients undergoing SACT and associated care. Working under the guidance and supervision of the CNS team, the coordinator will assist in facilitating and supporting the patient through their journey.
