A leading healthcare provider in the UK seeks an enthusiastic Cancer Navigator to support Urology cancer patients. The role involves managing patient pathways, conducting Holistic Needs Assessments, and collaborating with multidisciplinary teams to ensure quality care. Candidates should have strong communication and organizational skills, with a commitment to patient care. This is a full-time position based in Coventry.
We have an exciting opportunity for an enthusiastic, proactive person to join our Oncology Team as a Band 4 Cancer Navigator specifically to support our Urology Cancer patients through their Oncology treatment pathways.
The post holder will be required to work autonomously in managing patient initiated follow-up which includes maintaining the remote monitoring database. You will also need to be able to demonstrate excellent communication skills either over the telephone or face-to-face, where you will be responsible for carrying out Holistic Needs Assessments, sign-posting patients appropriately, escalation of complex issues to the Clinical Nurse Specialist Team as required and liaising with service users and multi-disciplinary teams.
The Oncology Cancer Navigator (Urology) will form an integral part of the multidisciplinary team in caring for the physical, social, and psychological needs of patients and carers diagnosed with urological cancer, throughout their treatment pathway. The post holder will have daily direct contact with patients and their families, providing support, advice, signposting, and assistance with the Living with and Beyond Cancer agenda. This will include the arranging and triaging of holistic needs assessments as part of their core role.
The post holder will work as part of the Urology multidisciplinary team under the guidance and supervision of a registered practitioner.
