Location: London. Primarily based from our Wimbledon head office with regional travel across your designated region (South London)Contract: Permanent, Full-TimeHours: Core Hours Monday to Friday 9am -5pm.Salary: £47,000NMC Registration: RequiredIdeal candidates will have, Lead Clinical Excellence in Community-Based Complex CareAt HFH Healthcare, we deliver comprehensive, nurse-led complex care to paediatric and adult clients in the comfort and familiarity of their own homes.Our care is delivered day-to-day by skilled carers, supported and clinically overseen by our expert nursing team. Guided by robust clinical governance, our services are designed to meet a wide range of complex health needs including spinal injuries, tracheostomy, and long-term ventilation.As an Onboarding Nurse, you will take ownership of the assessment, planning, and mobilisation of new care packages. You'll work collaboratively with the client, CHC teams, case managers, internal staff, and families to ensure every new package is set up safely, efficiently, and with quality at the core.If you're a Registered Nurse with community or complex care experience who thrives in a dynamic environment - and values person-centred care, flexibility, and strong clinical standards - we want to hear from you.What You'll Do

  • Complete clinical assessments and mobilisation planning for new packages
  • Oversee the safe onboarding of clients, ensuring carers are trained, competent and well-supported
  • Be visible in client homes during the first 4 weeks of care
  • Support and guide Nurse Case Managers through the onboarding process
  • Build trusted relationships with CHC teams, case managers, and external stakeholders
  • Represent HFH at meetings to promote our model supporting the development of referral pipelines

    Assessment and care planning experience
  • Trachy and vent experience is essential
  • Spinal knowledge is a plus
  • Must be a driver, Experience in complex care, ideally community-based
  • Strong communication, clinical oversight, and planning skills
  • Flexibility to travel and work outside 9-5 as needed
  • Passion for delivering personalised, safe, and responsive care, NMC registered Nurse
  • Enhanced DBS
  • Full UK driving licence and access to vehicle
  • Flexibility to travel across the Southern Region and work extended hours when needed
