Job Title: On Street Support- Housing Solutions

Location: Havering

Hours: 9am-5:30pm

Term: Temporary until Feb (with the possibility of extension)

Salary: 17.54 PAYE hour

In this role, you will play a vital part in helping individuals with multiple and complex needs prevent a return to rough sleeping. You will provide early intervention, ongoing support, and guidance following discharge from the hospital.

What you'll be doing:

• Offering guidance and developing support plans to ensure clients sustain their quality of life
• Engaging with various support agencies to promote personal independence and well-being
• Acting as the main point of contact for casework within the accommodation, coordinating solutions for drug and alcohol, domestic violence, and mental health issues
