Journey Hospice Central PA is looking for an experienced Hospice RN for On-Call Coverage 7 on / 7 off.

The Hospice RN will oversee a primary caseload of hospice patients and provide skilled professional nursing care to them, their families, and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, New Jersey Hospice licensure laws, accrediting body standards, and agency policies and procedures. Responsibilities include initial assessments, comprehensive nursing assessments, developing and implementing the interdisciplinary team plan of care, teaching and supervising patients, families, and staff, and coordinating with other team members to meet patient needs. Provides supervision to Hospice Aides/Homemakers.

Education/Training/Certifications:

  • Graduate of an accredited registered nursing program.
  • Bachelor’s degree in Nursing preferred.
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred.
  • PA RN Licensure by the PA State Board of Nursing in good standing.
  • Valid driver’s license and reliable transportation insured in accordance with PA state requirements.

Skills and Knowledge:

  • Maintain a professional attitude and effective communication skills, relating courteously and cooperatively with patients, caregivers, colleagues, supervisors, and others.
  • Demonstrate optimism, enthusiasm, and willingness to work constructively with health care team members, reflecting concern for patient and employee well-being.
  • Support the mission and vision of LSMNJ through positive interactions with staff.

Experience:

  • Minimum of one (1) year of general nursing experience preferred, with current hospice, home health, medical, surgical, or critical care experience.
  • Experience with pain and symptom management interventions preferred.
  • Ability to work within an interdisciplinary setting.
  • Understanding of hospice philosophy and principles of death/dying.
