Aneurin Bevan University Health Board is seeking an experienced practitioner to provide person-centred therapeutic support to older people with mental health challenges across community settings, care homes and memory services.

The role includes specialist assessments, memory and functional assessments, and delivering evidence-based care for dementia and other conditions, with a focus on least restrictive practice and risk management.

Qualifications

• Knowledge of functional mental health concerns.
• Knowledge of dementia types.
• Welsh language ability is desirable.

Responsabilités

• Provide person-centred therapeutic support to older people with mental health difficulties across community settings, care homes and memory services.
• Carry out specialist assessments, reviews and interventions for complex needs including dementia and cognitive impairment.
• Collaborate with GPs, OTs, psychologists and social care colleagues to coordinate care and support service development and quality improvement.
