Job summary

NHS Grampian is seeking a compassionate Community Mental Health Nurse to join the Older Adult Community Mental Health Team, Moray. You will work within a multidisciplinary team delivering person-centred care to older adults with functional mental illness, dementia and complex health needs in community settings.

Under supervision of senior clinicians, you will assess, plan and implement evidence-based interventions, coordinate care with families, carers and other services, and support ongoing

Qualifications

Registered Mental Health Nurse with post-registration mental health experience.
Experience in community settings desirable.
Driver and access to a car is essential.
Post is 22.5 hours per week, Monday to Friday.
Interviews scheduled 25 November 2026.

Responsibilities

Undertake comprehensive mental health assessments and contribute to risk assessment and management.
Develop and review care plans and deliver evidence-based interventions to promote recovery and wellbeing.
Manage a defined caseload and work with service users, families and carers to tailor care.
Collaborate with primary care, social work, care homes, inpatient services and third-sector organisations.
