Job description

Tripod Partners are looking for 2 Independent living Assessors (ILA)/ Occupational Therapy Assistant's to work for a council in Central London.

Responsibilities

  • Carry out functional assessments to evaluate service users' ability to manage daily living tasks safely and independently.
  • Coordinate and monitor cases which may include reablement programmes, setting goals and reviewing progress weekly to reduce dependency on services.
  • Hold a caseload with weekly allocations, ensuring timely and effective support planning and delivery.
  • Arrange and fit equipment which may include specialist equipment and identifying minor and simple major adaptations, to promote independence at home.
  • Support safe manual handling and transfers, under guidance from qualified OT and physiotherapy staff.
  • Develop outcome-focused support plans for individuals.
  • Process referrals and complete discharge assessments, identifying ongoing needs and eligibility under the Care Act 2014.
  • Liaise with multidisciplinary teams and external agencies, ensuring coordinated care and timely service delivery.
  • Promote the use of assistive technology, identifying opportunities to enhance independence and reduce reliance on statutory services.
  • Maintain accurate records and contribute to performance monitoring, safeguarding compliance and service quality.
  • Must have experience working as a Occupational Therapy Assistant
