What you will do:

Assess adults and children with disabilities, coordinating the provision of necessary equipment and adaptations to enhance their independence and safety. A solid understanding and experience with DFG is essential.

Conduct risk assessments for complex moving and handling situations and develop effective management plans.

Engage service users and carers actively during assessments, support plan developments, and facilitate access to Direct Payments.

Identify and demonstrate solutions, including assistive technologies, ensuring their safe usage.

Maintain accurate, timely assessment and case records for all service users, reviewing support plans for ongoing effectiveness.

Update systems to support service delivery and ensure the collection of statistical data.

Collaborate seamlessly with colleagues and other health and social care professionals, facilitating smooth transitions for service users. Engage actively in Adult Social Care developments and your professional growth.

Requirements:

Must be registered with HCPC with a minimum of 2 years of experience in this specialism.

Possessing a valid driving licence is preferable for this role.
