Overview

Occupational Therapist (OT) needed for Home Health in Richmond. Full-Time, Part-Time, or Per Diem. $5,000 Sign-on Bonus.

Health Link Home Health and Hospice provides comprehensive home health services in the greater San Francisco Bay Area and East Bay. We are seeking a compassionate, organized, and team-oriented OT to serve Richmond and surrounding areas. We offer a collaborative, supportive work environment with strong back-office support.

Responsibilities

• The Occupational Therapist will provide services within the scope of occupational therapy, following the plan of care individualized for the patient, and coordinating with the Interdisciplinary Team.
• The Occupational Therapist will follow the plan of care; provide treatment; evaluate the patient''s reaction to treatment and report any changes in condition to the Physician, Case Manager, and/or Director of Nursing.
• Maintain appropriate clinical records and/or notes and submit the records in accordance with organization policy.
• Be responsible for following all company policies and procedures.

Pay

Pay Rate: $65-$75 per hour DOE
