Job summary

Qualifications

• Registered OT with UK registration.
• Experience with adults 18+ in lifelong services.
• Experience with mental capacity assessments.

Responsibilities

• Conduct strengths-based reviews for adults 18+ with varying disabilities.
• Complete Mental Capacity Act assessments and CHC checklists.
• Develop and implement personalised support plans; advise on home adaptations.
• Utilise evidence-based practice to guide interventions and assess risk.
• Participate in Multi-Disciplinary assessments with specialist input.

What you will do:

• Conduct strengths-based reviews with a variety of adults, from those with physical disabilities to those with acquired brain injuries.
• Complete mental capacity act assessments and Continuing Health Care checklists.
• Develop and implement personalised support plans, offering advice and recommending home adaptations where needed.
• Utilize up-to-date evidence-based practice to guide your interventions and carry out necessary risk assessments.
• Participate in Multi-Disciplinary assessments using your specialist knowledge.
