Are you an experienced band 5 or currently a band 6 that is passionate about helping people leave hospital with the right support, at the right time, and in the right place? Do you thrive in fast-paced, team-focused environments where your clinical skills directly change lives?
Our Integrated Discharge Team (IDT) is at the heart of smooth patient transitions from hospital to home or other care settings. We bring together the expertise of health and social care professionals to make sure each discharge is safe, timely, and tailored to the individual's needs.
As a Band 6 Occupational Therapist, you'll use your specialist skills to assess functional needs, plan discharge pathways, and coordinate support that promotes independence and dignity. You'll work alongside a supportive, multi-disciplinary team that includes nurses, physiotherapists, social workers, and community partners - all committed to seamless, patient-centred care.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
