Our Integrated Discharge Team (IDT) is at the heart of smooth patient transitions from hospital to home or other care settings. We bring together the expertise of health and social care professionals to make sure each discharge is safe, timely, and tailored to the individual’s needs.

As a Band 6 Occupational Therapist, you’ll use your specialist skills to assess functional needs, plan discharge pathways, and coordinate support that promotes independence and dignity. You’ll work alongside a supportive, multi-disciplinary team that includes nurses, physiotherapists, social workers, and community partners – all committed to seamless, patient-centred care.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
• To assess, plan and coordinate safe, timely discharges via the most appropriate discharge pathway to meet the patients' required needs.
• Manage a caseload of patients with varying diagnosis and or /complex needs with a range of disabilities including physical, learning and mental health disabilities.
• To educate and train the team on the role of OT and provide day to day management for junior staff, ensuring supervision and annual review.
• To participate in the planning, development and evaluation of the discharge team service within the acute trust and the wider integrated services.
• Take a lead for information management to ensure high quality data which is recorded, evaluated & reported in a timely manner.
