Job summary

Sanctuary Personnel is seeking an experienced Occupational Therapist to join our team in Slough. This permanent role offers hybrid working and flexible hours to fit your lifestyle.

You will undertake holistic assessments in home and community settings, arrange equipment and major/minor adaptations, and contribute to discharge planning. You will collaborate with social workers, housing teams, health professionals, and external providers to promote independence and safeguard adults.

Qualifications

• Degree in Occupational Therapy with HCPC registration or eligible
• Experience in adult social care, rehabilitation or community settings
• Knowledge of moving and handling legislation and safeguarding adults

Responsibilities

• Undertake holistic occupational therapy assessments within home and community settings, identifying functional needs to promote independence
• Recommend and arrange equipment, assistive technology, and major/minor adaptations, including referrals where necessary
• Complete moving and handling assessments, providing guidance and risk management plans for carers and support providers
• Develop person-centred support plans in line with legislation and best practices, focusing on prevention and reablement
• Collaborate with social workers, housing teams, health professionals, and external providers to ensure integrated care and smooth discharge planning
