The NHS is seeking a passionate Occupational Therapist for the Stroke Early Supported Discharge Service in Great Yarmouth. This part-time role (21 hours/week, Band 6) involves managing a caseload, promoting patient recovery, and collaborating with a multidisciplinary team. Ideal candidates will have post-graduate experience in stroke rehabilitation and excellent communication skills. We have an exciting opportunity for an enthusiastic Occupational Therapist to join our established Stroke Early Supported Discharge Service. We work closely with acute stroke services in our area and our neurology colleagues in the community to provide continuity of care across the stroke pathway. Our aim is to promote recovery and well-being, whilst empowering patients to self-manage and maximise their independence using health coaching techniques and meaningful activity. We support safe discharges home and facilitate rehabilitation therapy in patients' places of residence.
We are looking for a qualified occupational therapist with post-graduate experience working with stroke patients, who can manage their own caseload, whilst collaborating with the multidisciplinary team and partner organisations.

We take a very holistic approach to rehabilitation, across the domains of self-care, productivity and leisure. You will have the opportunity to utilise your occupational therapy skills to the full. As well as conducting assessments and devising rehabilitation programmes aligned to patients' goals, we provide education, emotional support and carer support.
The immediate team includes occupational therapists, physiotherapists, a speech and language therapist and therapy assistant practitioners.

To communicate effectively with patients and carers tomaximise rehabilitation potential and to ensure understanding of their stroke.Communication skills of persuasion, motivation, explanation and gaininginformed consent will be used with a wide variety of patients. Barriers toeffective communication may include loss of hearing, cognitive difficulties,expressive and receptive aphasia, pain and fear.
To assess capacity, gain validconsent and have the ability to work within a legal framework with patients wholack capacity to consent to treatment.
To help facilitate patients toself-manage their rehabilitation.
To take responsibility for appropriate delegation oftasks.
To train, supervise and manageperformance of assistant practitioners, therapy assistants and students, withassistance from more senior members of the team. This will include the use of formalappraisal documentation.
To maintain up-to-date knowledge ofthe full stroke care pathway.
Professional
To maintain own clinical professional development(CPD) by keeping abreast of any new trends and developments and incorporatethem as necessary into your work.
To be an active member of the in-service trainingprogramme by the attendance and presentation at staff meetings, tutorials,training sessions, external courses and reflective practice.
To communicate effectively and workcollaboratively with medical, nursing and therapy colleagues to ensure deliveryof a co-ordinated multidisciplinary service.This will include case conference and medical reports. To provide teaching/training to other membersof the multidisciplinary team.
To participate in the staff appraisal scheme andpersonal development.
To undertake the measurement and evaluation of yourwork and current practices through the use of Evidence Based Practice projects,audit and outcome measures. To make recommendations for and assist with change.
To demonstrate a sound understanding of ClinicalGovernance and Risk Management and apply to work situation.
Organisational
To be responsible for a designatedarea of work, as agreed with the Team Leader. To plan and organise this efficiently withregard to patient management and use of time.
To decide priorities for own work,balancing other patient related, professional and service demands.
To be responsible for ensuring theeffective selection and use of all treatment resources.
To be responsible for equipment used in carrying outclinical duties, and to adhere to departmental policy, including competence touse equipment and to ensure the safe use of equipment by others throughteaching, training and supervision of practice including manual handling inaccordance with professional guidelines.
To maintain concise, accurate,comprehensive and up-to-date documentation in a timely manner in line withlegal and departmental requirements, including letters and reports asappropriate.
To be actively involved in thecollection of appropriate data and statistics for the use of the department.
To be aware of the Health and Safetyaspects of your work and implement any policies which may be required toimprove the safety of your work area, including the prompt recording andreporting of accidents / incidents to senior staff, and ensuring that equipmentused is safe.
To comply with the organisational anddepartmental policies and procedures and to be involved in the reviewing andupdating as appropriate.
