Job summary

Gateshead Council is seeking an Occupational Therapist to help adults with care and support needs live independently in their homes and communities. You will carry out holistic assessments, develop person-centred plans and deliver practical occupational therapy interventions within the Care Act 2014 and current legislation.

As part of Adult Social Care you will work with individuals, families and carers, recommending equipment and adaptations, and supporting reablement.

Qualifications

• HCPC registration is required.
• Experience in health or social care environment is desirable.
• Full UK driving licence or access to a car is required.

Responsibilities

• Plan, implement and evaluate services based on assessed risks and needs in line with Council policy.
• Prepare accurate reports and recommendations for minor adaptations, equipment and reablement plans.
• Provide practical advice on equipment and adaptations to users and carers to ensure safe use.
• Coordinate provision of equipment, reablement and instruct care providers in equipment use.
• Ensure accurate recording and data quality across all case records.
• Carry out occasional joint visits with partners for feasibility surveys and advice.
• Maintain knowledge of Council priorities and current legislation.
• Adhere to the College of Occupational Therapy Code of Ethics and CPD requirements.
