Qualifikationen

Aufgaben

• Carry out timely OT assessments in line with Care Act 2014 and local procedures.
• Assess functional needs and plan interventions to maximise independence and safety at home.
• Arrange equipment, adaptations and practical solutions to support daily living.
• Identify urgent/high-risk referrals and respond appropriately.
• Provide guidance on major/minor adaptations and moving and handling as required.
• Collaborate with Social Workers, Health Professionals and Housing colleagues.
