About the Role:
Pertemps Social Care is seeking an experienced Occupational Therapist to join Slough Council’s Discharge to Assess (D2A) team. This essential role involves supporting timely hospital discharges and applying professional clinical judgment to ensure safe and appropriate transitions of care.

Key Responsibilities:
  • Support and complete Trusted Assessments as required
  • Use clinical expertise to screen and process hospital discharges in line with D2A protocols
  • Work collaboratively with health and social care professionals to ensure effective discharge planning
  • Promote reablement and independence as part of discharge support
Requirements:
  • HCPC registered Occupational Therapist
  • Previous experience in D2A, hospital discharge, or intermediate care teams
  • Strong clinical reasoning and assessment skills
  • Understanding of current discharge pathways and community resources
