This is an excellent opportunity for an HCPC-registered Band 6 Occupational Therapist to work within the West Dunbartonshire Community Hospital Discharge Team, managing a complex clinical caseload of older and disabled adults with multiple pathologies. The focus of the service is to promote independence, support self-management, avoid hospital admission, and facilitate timely hospital discharge through person-centred care. The post holder will contribute to interdisciplinary assessments, case coordination and service delivery, while also engaging in supervision of staff and students, health promotion, audit activity and service development.
This community-based role requires travel across West Dunbartonshire; therefore, a full UK driving licence and access to a car is essential. Experience of working within a multidisciplinary team and knowledge of community practice, older people's care, or neurological conditions is desirable.
