Your role will include participation in our duty system, community hospital discharge planning, and case management.

If you have completed Council Officer Training, you will also be part of our Adult Support and Protection rota; if not, we will support you in completing this within an agreed timeframe. We are seeking a compassionate and empathetic professional who is passionate about delivering high standards of practice and committed to working collaboratively with colleagues to support individuals who rely on our services.
