This position is based within the Hospital Discharge Team, a small dynamic team working closely with partner agencies to ensure assessments are completed in a timely manner, facilitating the flow out of hospital sites.

Experience in this area is preferable but not essential. Applications are welcome from Social Workers, Occupational Therapists, and Nurses.

Responsibilities
  • Provide a statutory Social Work Service in accordance with relevant legislation and guidelines.
  • Manage cases by assessing service users' needs and risks, utilizing accredited risk assessment tools when necessary.
  • Follow service procedures, deliver direct interventions to individuals and small groups, participate in supervision sessions, and represent the Social Work Service in multidisciplinary forums.
  • Work in accordance with the Council or stakeholder arrangements through Service Level Agreements or Partnerships.
