Fixed Term Until 30/09/2026
This position is based within the Hospital Discharge Team, a small dynamic team working closely with partner agencies to ensure assessments are completed in a timely manner, facilitating the flow out of hospital sites.
Experience in this area is preferable but not essential. Applications are welcome from Social Workers, Occupational Therapists, and Nurses.
