Leeds Community Healthcare NHS Trust in Leeds is seeking a Band 6 Senior Occupational Therapist to join the Wharfedale Recovery Hub, delivering person-centred rehabilitation to adults and supporting timely discharge from short-term care beds.

Band 6 Senior Occupational Therapist – Short Term Care Beds.

An exciting opportunity has arisen for a Band 6 Occupational Therapist to join the team at Wharfedale Recovery Hub.

Wharfedale Recovery Hub provides short-term rehabilitation for individuals who no longer require acute hospital care but are not yet able to be supported safely at home. We are seeking a dynamic and motivated Occupational Therapist to deliver high-quality, person-centred rehabilitation to adults, supporting their recovery and preparation for discharge home.

The role offers a varied and rewarding caseload, including the management of long-term conditions, falls prevention, and facilitating timely hospital discharges. This diversity provides excellent opportunities to develop a broad and transferable skill set. You will work holistically with patients, using health coaching and self-management approaches to support them in achieving their personal goals.

You will be part of a supportive multidisciplinary team, working collaboratively, drawing on shared expertise, and appropriately delegating or signposting to other professionals to ensure the best possible outcomes for patients.

You will deliver and facilitate high-quality rehabilitation to support patients in achieving their individual goals, maximising opportunities for independence and enabling them to live meaningful and fulfilling lives. Working in partnership with patients and their families, you will undertake comprehensive assessment, planning, implementation and evaluation of treatment and care programmes, using person-centred and strengths-based approaches to promote involvement, choice and empowerment.

You will support recovery and optimise independence by working collaboratively as part of a multidisciplinary team, contributing specialist occupational therapy expertise to ensure holistic, coordinated care.Through effective communication, goal-setting and shared decision-making, you will help patients progress safely towards discharge while promoting self-management, confidence and long-term wellbeing.
