Purpose of job

The Post holder will be an integral part of the Occupational Therapy & Sensory Services, delivering high-quality services to support adults using a person-centred approach. They will focus on well-being, prevention, choice & control for individuals, maximising their independence and potential.

The Occupational Therapist will meet with Service Users, carers, families, multi-disciplinary agencies, and organizations (in various locations as required) to enhance independence, safety, and quality of life through rehabilitative planning, equipment specification, environmental modifications, and support plan development. They will coordinate assessments, reports, and presentations to multi-agency panels.

They will ensure compliance with Council policies/procedures and statutory obligations, adhering to the College of Occupational Therapy Code of Ethics & Professional Conduct and HCPC Standards of Proficiency.

Responsibilities to the Advanced Practitioner

Responsible for the quality and prioritization of their work, alerting Line Management to workload issues, and maintaining performance standards and adherence to policies and targets.

Main duties of the post
  1. Assess the needs of Service users, formulate care plans, and arrange service provision, including rehabilitative guidance, adaptations, equipment, and advice.
  2. Participate in multi-professional meetings, panels, and forums as required.
  3. Undertake assessments in line with legislation, policies, procedures, and eligibility criteria.
  4. Arrange for the provision, installation, and demonstration of equipment and adaptations, maintaining up-to-date knowledge of specialist equipment and services.
  5. Identify, alert, and refer complex or safeguarding issues to the appropriate social work team.
  6. Manage a caseload, identify and reduce risks, and coordinate equipment and adaptation provision.
  7. Assess carers and work in partnership with other agencies and organizations.
  8. Create and maintain accurate service user records following policies and professional guidelines.
  9. Undertake Moving and Handling risk assessments and provide equipment and training.
  10. Manage conflicts professionally and resolve contentious issues.
  11. Maintain professional competence through CPD and contribute to service development.
  12. Perform other duties appropriate to the role, including service and staff development.
  13. Adhere to Health and Safety, equal opportunities, customer care policies, and technology agreements.
  14. Supervise and allocate work to up to 2 staff members, accountable to the Team Manager.
  15. Maintain a professional portfolio for CPD and HCPC requirements.
  16. Participate in the transition process for young people with disabilities to adulthood, liaising with relevant departments and agencies.
  17. Comply with financial, personnel, administrative, and management information systems as directed.
  18. Participate in Duty/Enquiry systems, prioritizing work in consultation with managers.
  19. Respond to and collate information related to complaints and service issues as requested.
