As an Occupational Therapist within the organisation, you will play a pivotal role within a multi-disciplinary framework, providing essential health and social care services to the local community.
Key Responsibilities:
- Care Coordination: Lead the development and coordination of care and support plans for service users, their families, and carers, leveraging the expertise of other professionals as required.
- Comprehensive Assessment & Intervention: Utilise evidence-based occupational therapy methodologies for the thorough assessment, targeted intervention, meticulous planning, and ongoing review of complex cases.
- Promotion of Independence: Proactively identify and maximise opportunities for service users' re-enablement, fostering sustained independence and optimising their choice and control over their lives.
- Inter-agency Collaboration: Engage in collaborative practice with both internal departments and external agencies, providing expert advice and disseminating information regarding available resources.
- Regulatory Adherence: Ensure all duties are executed in strict accordance with the Care Act 2014, the Mental Capacity Act 2005, and the HCPC Standards of Proficiency.
- Professional Development: Maintain continuous professional development and registration, adhering to the guidelines set forth by the College of Occupational Therapists' Code of Continuing Professional Development.
Candidate Requirements:
- A Diploma in Occupational Therapy or an equivalent, recognised qualification.
- Substantial post-qualifying experience as an Occupational Therapist, ideally gained within Adult Social Services or a comparable community health setting.
- Current and valid HCPC registration.
- A comprehensive understanding of relevant health and social services legislation, particularly the Care Act 2014 and Mental Capacity Act 2005.
- Demonstrable excellence in written and oral communication, IT proficiency, report writing, negotiation, and advocacy skills.
- The proven ability to function effectively within a multi-disciplinary team environment and to manage a demanding caseload with efficiency.
- For candidates aspiring to higher grades (PO2-PO3): Experience in conducting Best Interest Assessments (BIAs), providing mentorship to junior colleagues, and contributing to professional training initiatives will be highly regarded.
