Job description

An exciting opportunity has arisen for a compassionate and skilled Occupational Therapist to join our dynamic multidisciplinary team within the NHS in the Crewe area. As an Occupational Therapist, you will play a pivotal role in assessing and supporting patients to improve their independence and quality of life, whether in a hospital, rehabilitation unit, or community setting. Working closely with other healthcare professionals, you will deliver person-centered care, ensuring patients achieve their maximum functional potential.

Key Responsibilities:

  • Patient Assessment: Carry out comprehensive assessments of patients' physical, emotional, and cognitive abilities to identify their needs and challenges.
  • Care Planning and Intervention: Develop, implement, and evaluate individualized treatment plans to improve patients' independence in daily activities.
  • Therapeutic Activities: Facilitate interventions using evidence-based approaches, including the provision of assistive devices, environmental adaptations, and rehabilitation exercises.
  • Patient Education: Educate patients and their families/carers about managing daily tasks, adapting to life changes, and using assistive technology where appropriate.
  • Multidisciplinary Collaboration: Work alongside physiotherapists, nurses, social workers, and doctors to deliver coordinated and holistic care for patients.
  • Home Visits (if required): Assess patients’ home environments and recommend modifications or equipment to enhance safety and independence.
  • Documentation and Reporting: Ensure all patient records, reports, and care plans are completed accurately and in line with NHS guidelines.
  • Professional Development: Participate in ongoing professional development, including training, supervision, and reflective practice, to maintain high standards of care.
  • Service Improvement: Contribute to audits, service evaluations, and quality improvement initiatives to enhance service delivery within the team.

Degree or diploma in Occupational Therapy and current registration with the Health and Care Professions Council (HCPC).

Experience in a clinical setting, ideally within the NHS, with knowledge of rehabilitation and long-term care needs.

Strong communication and interpersonal skills to work effectively with patients, their families, and multidisciplinary teams.

Ability to manage a varied caseload and prioritize workload in a busy clinical environment.

A compassionate approach, with a focus on empowering patients to maximize their independence.

Strong organizational skills and attention to detail, with the ability to maintain accurate patient records.
