Exciting opportunities have arisen across Liverpool. We are seeking a dynamic and compassionate Occupational Therapist to join our multidisciplinary team, with flexible rotational opportunities across both community and bed-based services.

The ICRAS service delivers urgent and short-term reablement and assessment support to individuals either at risk of hospital admission or transitioning out of acute care. Our integrated model promotes admission avoidance, safe discharges, and person-centered care, working in partnership with social care providers, GPs, community clinicians, and other stakeholders.

Key Responsibilities:

  • Deliver high-quality, patient-centred assessments and reablement interventions in community and inpatient settings.
  • Work autonomously and collaboratively within a multidisciplinary team including Advanced Practitioners, Nurses, Pharmacists, Social Workers, Dietitians, and other allied health professionals.
  • Support patients in their own homes or rehab units to regain independence, reduce the risk of hospital admission, and promote safe and timely discharge.
  • Participate in the development, implementation, and evaluation of individualised care plans.
  • Contribute to service development and quality improvement initiatives.
  • Provide supervision and support to junior staff and students as required.
  • Engage with family members, carers, and external agencies to ensure holistic care planning.

Requirements:

  • Qualified and HCPC registered Occupational Therapist or Physiotherapist.
  • Demonstrated experience in a relevant clinical setting (community or acute).
  • Ability to work independently across a range of clinical environments.
  • Full UK driving license and access to a vehicle for work purposes.
  • Excellent communication, decision-making, and interpersonal skills.
  • Flexible and resilient, with the ability to adapt to a changing and dynamic service.
