Job summary

CSH Surrey, part of NHS community services, is seeking a Nursing Associate to join its community nursing teams. The role works under indirect supervision of a Registered Nurse, delivering holistic care to patients in the community and supporting HCAs and trainees.

Job summary

If you want to be part of this exciting challenge then consider coming to work with CSH Surrey. We are looking for talented, forward- thinking Nursing Associates to join our Community Nursing services.

The successful applicant will be supporting the Nursing workforce to provide safe and effective care under the indirect supervision of a Registered Nurse (RN). You will be working within the scope of your practice, providing, holistic nursing care to our patients living within a defined geographical location within the community. You will support and supervise HCAs, Trainee Nursing Associates and Pre-registration nurses. You may also be expected to participate in service improvement initiatives and audits. This role will continue to develop through the acquisition of further skills based on the Nursing and Midwifery Council (2018) guidelines and the development, going forwards of the Nursing Associate role.

Main duties of the job

The main duties of the Nursing Associate role are to

  • Deliver clinical care to patients as required within set protocols and within the scope of the Nursing Associates practice.
  • Contribute to patient assessment (after appropriate delegation from the RN) and plan and deliver high standards of care.
  • Co-ordinate a team of Health Care Assistants in care delivery and, where appropriate, other students
  • Take responsibility for a group/caseload of patients as required and when delegated by a RN.
  • Delegate tasks (not responsibility) to other members of the clinical care team as appropriate, e.g. Healthcare support workers, family members and lay carers.
  • Assist in the delivery of health promotion advice as appropriate to role and in line with own level of competence.
  • Undertake agreed delegated enhanced tasks as specified in local Standard Operating Procedures (SOPs), competencies under the indirect supervision of an RN.

Job responsibilities
Communication:

Planning and Organisational Skills:

Assist in the ordering, checking, unpacking and storing of items delivered to the clinical area and follow up orders as appropriate.

Patient/Client Care:

Act at all times in a manner, which illustrates respect for privacy, dignity and confidentiality. Provide nursing care that is safe and compassionate.

Provide compassionate, safe and effective care and support to people in a range of care settings.

Monitor the condition and health needs of people within their care on a continual basis in partnership with people, families, and carers.

Undertake and perform clinical skills to enhance the delivery of patient care, this will include Venepuncture and Removal of clips and sutures.

Measure, monitor & record the following vital signs:- Temperature, Blood pressure, Pulse, Respiration, SATs, Neurological observations, Pain score, and reporting and escalating appropriately on NEWS scoring.

Perform and record blood glucose levels utilising blood glucose monitoring equipment reporting adverse results to RN.

Insert urinary catheters as per CSH Policy under the indirect supervision of a RN.

Perform and record urinalysis as per CSH policy, reporting adverse results to RN.

Administration of medication under the indirect supervision of a RN upon successful completion of competencies, and in line with CSH policy and the NMC (2018) Standards of proficiency Annexe B Procedures Standards of Proficiency. Via the following routes: oral; intra Muscular Injections; subcutaneous injections; topical; inhaled; per rectal/vaginal and enteral.

Obtain requested specimens and ensure transportation to appropriate department Faeces, Sputum, and Exudates e.g. swabs nose, exit site, wound site, etc.

Perform basic life support, give assistance to professionals during advanced life- support techniques.

Contribute to the safe and timely discharge of all patients.

Assist in the delivery of health promotion advice as appropriate to role and in line with own level of competence, e.g. Smoking Cessation and diabetes management.

Contribute to the provision of care for people, including those with complex needs.

Understand the roles of a range of professionals and carers from other organisations and settings who may be participating in the care of a person and their family.

Policy and Service development:

Maintain professional behaviour and attitude at all times, and demonstrate excellent customer care skills in line with the CSH Values

Adhere to CSH Uniform Policy.

Understand and adhere to CSH Zero Tolerance (Violence and Aggression) policy.

To be aware of, and demonstrate adherence to, CSH policy on infection control and waste disposal. To adhere to the principles of universal precautions in all aspects of patient care.

Ensure all equipment is cleaned after use in line with CSH Policy.

Complete CSH competencies and Preceptorship programme.

Adhere to the recommendations for the Control of Substances Hazardous to Health (COSHH) and risk assessments appropriate to the clinical area.

Engage in public health, community development, and in the reduction of health inequalities.

Be familiar with, comply and be able to access all CSH Policies and Procedures

Human Resources:

Support and supervise others e.g. health care support workers, apprentices - including Trainee Nursing Associates.

Research and Development:

As directed by manager undertakes audits in the clinical area and participates in research where appropriate.

Freedom to Act:

The Nursing Associate will be proficient and competent and work within their scope of practice as governed by the NMC Code (2018) and NMC Standards of Proficiency ( 2018).

Act as a role model by upholding and implementing good practice in the workplace, always ensuring the highest standards of evidence based care.
