Job summary

Oxleas NHS Foundation Trust is seeking a qualified Nursing Associate (Band 4) to join our Rapid Response Team in London. Qualifications

• Registered Nursing Associate with NMC registration.
• Completed Nursing Associate programme.
• Driver's license (car driver) preferred.

Responsibilities

• Deliver delegated assessment responsibility and care plans under supervision.
• Participate in delivering complex care as prescribed by a registered clinician.
• Plan and document patient progress on Trust information systems.
• Maintain accurate and current records in line with professional codes.
• Evaluate patient progress and discuss with senior clinicians in MDT settings.
• Demonstrate use of equipment to maximise patient independence.
• Educate patients and carers to promote self-care and independence.
• Contribute to team/service development to improve patient care.
