Provide CIC in Chelmsford is seeking a Nursing Associate/Practitioner to join the Urgent Community Response Team (UCRT). You will perform in-home assessments and deliver high-quality, patient-centered care to help people stay safely at home and avoid hospital admissions.
Working 37.5 hours across a 8am–10pm seven-day service, you will need a Foundation Degree, an NMC Pin, and experience in community/acute settings. The role includes autonomous work with training provided.

Join our Urgent Community Response Team (UCRT) and help deliver high-quality, patient-centred care in the community, supporting people to remain safely in their own homes and avoiding hospital admissions.
UCRT is a 7-day service providing rapid assessment and treatment for patients experiencing a health crisis in their own homes, including nursing and residential care homes. Working closely with partners across Mid and South Essex—we provide holistic assessments within two hours of referral and deliver timely, coordinated care.
As a Nursing Associate/Practitioner, you will work both independently and as part of a skilled multidisciplinary team, assessing and treating patients in their own homes. Your role will include clinical assessments, monitoring observations, venepuncture, catheter care, supporting patients following falls, and managing acute episodes of illness. You will also carry out basic equipment assessments to support our Allied Health Professional colleagues, with full training provided.

UCRT operates 7 days a week between 8:00am and 10:00pm. The role includes weekend and bank holiday working on a shared rota.
The post holder will provide support to the members of UCRT which include Nurses, Allied Health Professionals and healthcare support workers to ensure that holistic assessments are completed whilst working collaboratively with the team to support and facilitate the treatment of patients within their own home.
The postholder will be required to provide a high quality, efficient and effective service to patients that are referred to UCRT, this means delivering acute care which could include simple reviews of patients on the UCRT caseload, monitoring observations, venepuncture, catheter care/catheterisation assisting patients that have fell with appropriate equipment and to support patients during an episode of ill health within their own home.
The post holder will be proficient, knowledgeable and competent and expected to work with minimal supervision, adhering to organisational policies and procedures and in line with their scope of practice competency. They will continue to develop in their role through the acquisition of further skills, knowledge and competencies to be determined by the needs of the patients, service and team.
The role of Nurse Associate/ Practitioner is intended to provide fully integrated, holistic care and support to ensure people are safely managed at home.
