Job summary

The Isle of Wight NHS Trust is seeking a registered Nursing Associate to work within Mental Health Services. You will practise in accordance with the NMC Code and Trust policies, supporting patients at home and in local services to improve care closer to where people live.

You will undertake risk assessments, refer as needed, and collaborate with patients, carers and partner agencies to ensure safe, effective care.

Qualifications

• Registered Nursing Associate and adherence to NMC Code.
• practice in line with Nursing Associate Standards of Proficiency and Trust policies.
• recognise and work within limits of competence to provide safe care.

Responsibilities

• Support people referred for assessment, support and/or treatment.
• undertake risk assessments and complete referrals/inter-referrals.
• plan and deliver intensive in-reach support with multidisciplinary teams.
