Job summary

East Lancashire Hospitals NHS Trust is seeking an experienced Nurse Associate to provide planned clinical care to housebound patients, including end-of-life care and wound management, across rotating day and night shifts. You will work within a multidisciplinary team, following the NMC Code and participating in professional development.

The role requires a Level 5 Foundation Degree in Health & Social Care and a full UK driving licence with access to a vehicle, supporting patients with complex

Qualifications

• Level 5 Foundation Degree in Health & Social Care (RN Associate) or equivalent.
• Evidence of recent study and application in the work environment.
• Minimum 12 months' experience in a healthcare setting.

Responsibilities

• Deliver clinical nursing care to housebound patients.
• Provide end-of-life care.
• Manage wounds.
• Support people with complex long-term conditions.
• Review planned care and elevate changes or concerns to registered nurses promptly.
• Work closely with patients, families, carers and colleagues to keep care holistic and person-centred.
• Build effective relationships with clinical teams and other professionals.
• Work across day and night shifts as the service requires.
• Contribute to service development and continuous improvement.
• Pursue ongoing professional development.
• Meet the NMC Code and the organisation's standards, behaviours and values.
