This role focuses on providing intensive home treatment for older adults, ensuring timely discharge from inpatient care and supporting clinical teams. An exciting opportunity has arisen for a B6 Mental Health Practitioner to join a newly created Home Intensive Treatment Service for older adults in Bury.
The team will provide an alternative to inpatient admission for Bury older adults with either a functional or organic condition by providing intensive treatment in the service users own home.
The team will also work to support clinical teams providing inpatient care to Bury older adults during the discharge process to ensure timely discharge and risk management and provide 72 hour follow up in the community.
The service will also provide a gatekeeping function to ensure prioritisation of referrals for inpatient admission and assessment of appropriateness for home treatment as an alternative.
The successful candidates will have the opportunity to contribute to the implementation and development of this newly established service in Bury.

-Providing intensive treatment in the service users own home as an alternative to inpatient admission.
- Supporting inpatient ward clinical teams during the discharge process to ensure timely and safe discharge of older adult Bury residents.
- To provide a gatekeeping function for all referrals requesting inpatient admission to ensure prioritisation and triage to ascertain if home treatment can be provided as an alternative to admission.
- To provide 72 hour follow up for all service users discharged from hospital.
