Job summary

Pennine Care NHS Foundation Trust is recruiting a Nurse Associate to deliver physical health checks within the Early Intervention team and to work with the Treatment/Physical health teams. The role includes home visits in the community and clinic-based duties, contributing to high-quality mental health and learning disability services across Greater Manchester.

You will perform physical health assessments, BP monitoring, venepuncture and administer depot medication, liaising with psychiatrists,

• Perform physical health checks including BP monitoring and venipuncture.
• Administer depot medication as part of treatment.
• Collaborate with Early Intervention and Treatment teams to support patient care.

Responsibilities

• Perform physical health checks in community and patients' homes.

Job description

The post holder will work primarily with the Early Intervention team 4 days a week completing physical health checks but work with the Bury Treatment team for 1 day a week doing Depot injections and working in Clozapine clinics. Performing Physical health checks for patients open to the Early Intervention service working closely with the Consultant Psychiatrist and Team manager along with care coordinators for the client group. The post is both clinic based in the community and in patients own homes.
