Job summary

Buckinghamshire Healthcare NHS Trust is recruiting a Nursing Associate to join the Marlow Adult Community Healthcare Team. This full-time post offers a supportive environment with a preceptorship programme and structured orientation to help you develop competencies.

You will assist community nurses with admission prevention, discharges, end-of-life care, and do holistic patient assessments, wound care, and medication administration.

Qualifications

  • Registered Nursing Associate on the NMC register.
  • Minimum 2 years’ experience in health care.
  • Experience in working in multi-disciplinary teams.

Responsibilities

  • Provide compassionate, safe and effective care and support to people in a range of care settings.
  • Contribute to ongoing assessment of patients, referring to a Registered Nurse when needed.
  • Monitor patients' condition with families and carers.
  • Record all care clearly, concisely and timely; sign and date records.
  • Communicate effectively with colleagues, sharing information and handing over responsibility.
  • Communicate with patients, offering care and advice.
  • Recognise and report situations that could affect care outcomes.
  • Assist with discharge planning and transfer paperwork.

Job overview

An exciting opportunity has arisen for a full time Nursing Associate to join the Marlow Adult Community Healthcare Team.

The successful candidate will be part of the friendly and proactive community team. Working in collaboration to provide a high standard of nursing care in the community.

We offer a preceptorship programme for newly qualified nursing associates, an extensive orientation programme, with the support of the advanced community practitioners and the practice development team.

Training and support will be given to achieve your competencies.

The role will be supporting the Community Nurses in a wide range nursing activities. These include admission prevention, facilitating discharges from hospital and end of life care. It will involve amongst other tasks holistic patient assessment, medication administration, wound and pressure area care and catheter management.

Main duties of the job
  • Provide compassionate, safe and effective care and support to people in a range of care settings.
  • Contribute to ongoing assessment of patients, recognising when it is necessary to refer and elevate to a Registered Nurse for reassessment
  • Monitor the condition and health needs of people within their care on a continual basis in partnership with people, families and carers
  • Record all care delivered appropriately and contemporaneously. Documentation must be clear, concise and legible. Any documentation must also be signed and dated.
  • Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care.
  • Communicate effectively with patients when delivering cares, providing advice where appropriate.
  • Recognise and report any situations, behaviours or errors that could result in poor care outcomes
  • Understand, encourage and complete the necessary paperwork to enable discharge/transfers in a safe and timely manner.

EDUCATION, QUALIFICATIONS & TR
  • Registered Nursing Associate on the NMC register
  • GCSE Grade A-C/Level 4-9 in Maths & English or skills functional level 2 Maths & English or equivalent
EXPERIENCE
  • Minimum 2 years’ experience in health care.
  • Experience of working in teams under appropriate supervision as part of a multi-disciplinary team
  • Experience of providing and receiving complex, sensitive information
