Job overview

An exciting opportunity has arisen for a Nursing Associate to work in the Stamford Adult Community Mental Health Team.

The role will be part of an integrated health/social care team providing care for people experiencing severe and enduring mental health problems in the local community. The post will offer nursing interventions in a wide variety of clinical, therapeutic and social forms with the aim of promoting health, wellbeing, self-autonomy and independence. Stamford CMHT are part of the developing transformation service in our local area. The post holders will be required to work effectively as part of the multidisciplinary team in the delivery of high quality mental health care planning and risk management. This work will involve patients and their carers in the delivery of evidence based interventions/treatments, recovery focused work. The post holder will have a breadth of knowledge across the lifespan and across the fields of nursing, providing holistic and person centred care and support within the adult community setting. The nursing associate will work independently under the leadership of the registered nurse, working within the sphere of nursing and care and within all aspects of the nursing process.

Experience in working in a mental health setting is essential.

Detailed job description and main responsibilities

1. Provide therapeutic interventions with a service user.
2. Monitor the condition and health needs of people within their care on a continual basis in partnership with service users, families and carers.
3. Contribute to ongoing assessment, recognising when it is necessary to refer to others for reassessment.
4. Perform nursing interventions advocated within the service users care plan.
5. Recognise the contribution of others in the provision of service user care.
6. Involve service users/ carers in the planning and delivery of care in a compassionate and appropriate manner which considers their diverse needs.
7. Maintain the welfare and safety of service users and protect them from any form of abuse in accordance with safeguarding guidance.
8. Monitor the impact and effectiveness of nursing care delivered.
9. Demonstrate a knowledge of information governance and the need to protect service user confidential information.
10. Make and maintain accurate records of nursing care in accordance with the professional (NMC) guidance and Trust policy.
11. Demonstrate problem solving skills by identifying solutions in routine clinical issues.
12. Participate in regular clinical supervision.
13. Engage in public health, community development and in the reduction of health inequalities.
14. Be able to identify role boundaries.
15. Medicines Management as delegated by the Nurse in Charge and within the scope of practice and competency of the designated practitioner.

Person specification

Skills

• Insight into how to evaluate own strengths and development needs, seeking advice where appropriate
• Evidence of time management skills and ability to prioritise delegated workload
• Ability to communicate effectively with patients/ relatives and carers and all members of the multi-disciplinary team
• Ability to deal with non-routine and unpredictable nature of the workload and individual patient contact
• Ability to develop effective and appropriate relationships with people, their families, carers and colleagues
• Ability to support, supervise, assess and act as a role model to nursing associate students, other learners and health care support workers as required within the clinical setting
• Understanding of basic physiology, e.g. normal vital signs, fluid balance nutritional requirements etc.

Experience

• Previous experience of working within a health / care setting
• Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development
• Understands and acts in line with NMC professional standards for practice contained within The Code.
• Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
• Understand requirements for NMC professional Revalidation
• Understanding of the importance of following procedures and treatment plans
• Understanding of evidence based practice
• Experience of working with people with a mental health diagnosis

Qualifications

• Registered Nursing Associate on the NMC register
• GCSE Grade A-C or Grade 9-4 in Maths and English or functional skills level 2 Maths and English or equivalent
