Part time post 20hours working between 18:30-23:30 over 7 day rota based at Kidderminster
This is arole as a Community Nursing associate as part of the evening and night teamworking 18:30-23:30 over 4 evenings a week
The teamis a countywide team which is centrally based in Droitwich with satellite basesin Worcester, Evesham, Bromsgrove and Kidderminster, they visit patients intheir own home and support them in remaining at home and preventing admissionto hospital and supporting early discharge from hospital. This post will beprimarily based in the North of the county from either Bromsgrove orKidderminster.
The postis all worked as unsocial hours hence picking up pay enhancements for all hoursworked which makes this post more attractive. more money for less hours!!

As a nursing associateworking with the Evening and night Team most of the work allocated to you willof an unplanned nature. The service works as an on-call service covering theCounty of Worcestershire taking direct referrals from patients, 111, AmbulanceService and discharging hospitals. You can expect to have a comprehensiveinduction and work along side peers until you are confident to then work alone.
The evening service nurses primarily work alone, theyhave work allocated via a handheld tablet by the triage nurse based atDroitwich. You will be able to start your visits from home if the geographicsdictates.
Thepatients that are seen during the evening and night vary immensely and no 2shifts are the same. For example, we see catheters, palliative care, woundcare, administration of medication, diabetics, and patients requiring intensivesupport rehabilitation following discharge from hospital or admissionprevention.
As part of a new initiative, we are workingclosely with West Midlands ambulance service and 111 and assist in respondingto urgent Care response. These can be patients that have fallen, or patientsrequiring further assessment to aid prevention in admission to hospital.

You will bejoining the Evening element of one of our dynamic and supportive NeighbourhoodTeams to provide a 24-hour admission avoidance service to the population ofWorcestershire which encompasses both urban and rural areas.
The successfulcandidate will deliver independently a high standard of nursing care forpatients in their own homes. Ensuring that care provided maintains theindividuals privacy and dignity, values diversity and respects cultural andindividual rights and choices.
We are lookingfor candidates with good communication skills, a flexible approach andenthusiasm to work within the team. Youshould be a committed, highly motivated and forward-thinking individual who isa good team player. Community experience is desirable.
The successfulcandidate will also assist the team to develop and achieve team andorganizational objectives. They will also participate in staff meetings andtraining & development activities as necessary to ensure their skills andknowledge are up to date.
Due to thenature of the post the applicant must be able to travel independently and haveaccess to a suitable vehicle for business purposes. Whilst a base will beidentified, staff can be asked to work anywhere within the Business Unit orService if necessary.
