A healthcare service provider in the East Midlands is seeking a Nursing Associate to deliver high-quality patient care. This role involves working with housebound and care home patients, contributing to health management projects, and supporting Student Nursing Associates. Ideal candidates will have GCSEs in Maths and English, and preference is given to those with a Nursing Associate qualification or who are registered on the NMC. We have an exciting opportunity for a Nursing Associate to join our dynamic team. We are looking for someone who is enthusiastic about providing high quality care. The role will involve working with housebound patients, care home patients and contributing to PCN wide Population Health Management Projects. While a qualified Nursing Associate would be preferred, we would also be interested to hear from any potential candidates who are currently Student Nursing Associates, or anyone interested in becoming a Student Nursing Associate who is interested in this post. PICS is supporting Student Nursing Associates with their University studies through an apprenticeship.

Newark PCN has a patient population in excess of 79,000. You will be part of a wider team of Student/Nursing Associates and supported by our Lead Nurse who is on the PICS leadership team.

KEY REQUIREMENTS
GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent must have certificates
Nursing Associate foundation degree qualification (preferred, but Student NAs will be considered)
Registered Nursing Associate on the NMC register (preferred, but Student NAs will be considered)

Promoting health and preventing ill health
Support patients to improve and maintain their mental, physical, behavioural health and wellbeing, demonstrated in such as:
using behaviour change interventions to support health and wellness when undertaking health checks (physical and mental) which may include for example new patient checks, NHS Health checks (Q risk), over 75s checks and referring to other services as needed including signposting patients to personalised care colleagues and local community and voluntary sector services.
Chronic disease management contribute to the monitoring of patients with, for example, respiratory disease, diabetes, coronary heart disease, renal failure, hypertension.
Encourage patients to take part in health screening. The role may require, when appropriately trained, to perform cervical screening tests.
Care for individuals with complex conditions such as dementia, mental health conditions, and learning disabilities.
Providing and monitoring care
Carry out procedures safely and effectively within the PCN practices and also in the patients home (including care homes), clinical tasks to include:
Clinical observations such as BP / Pulse / Temperature / Oxygen saturations / point of care blood glucose / BMI
Venepuncture
ECG
Diabetic foot checks
Sample processing and management
Provide care and support with hygiene and maintenance of skin integrity including minor and complex wound care
Administration of medicines via oral, topical and inhalation routes and administer injections using subcutaneous and intra-muscular routes. This will include immunisations influenza / shingles / pneumonia, B12
Assist GP / ANP with minor surgery / invasive procedures for example coil fitting
Working in teams
Improving safety and quality of care
Cleaning and restocking consulting rooms, stock control of consumables.
Adhering to infection control policies and procedures to include participation in regular audits, hand hygiene training, vaccine/cold chain storage awareness, spillage control procedures.
Contributing to integrated care
