Overview
The main duties of this role will involve working with a Community Learning Disabilities Nurse; providing education on annual learning disability health checks to all relevant stakeholders. Working with GP practices in West Norfolk to support in improving the uptake and quality of annual learning disability health checks. Promote the use of pre-health check questionnaires and health action plans. Support with the running of health promotion groups to provide health promotion education and education on annual LD health checks to Adults with learning disabilities. Compiling a monthly report of the progress of work each month. You will work within an MDT. You will be line managed by the Health Team Manager and clinically supervised by the B7 Clinical Lead Nurse. You will support and possibly clinically supervise a band 5 Nurse or Nursing Assistant in the North LD team, based at Kelling Hospital. You will develop working relationships with other key stakeholders from outside the organisation, including care providers and acute hospitals. You will communicate with all members of the Community Learning Disability Team and the wider Integrated Care System including acute and Primary Care Networks, to ensure a cohesive approach to care delivery. You will work in partnership with people with lived experience, including carers and family. You will work jointly with other professionals within a range of disciplines, including social care.
Responsibilities
- Provide education on annual learning disability health checks to stakeholders.

- Support the running of health promotion groups to educate adults with learning disabilities about health checks.
- Compile a monthly progress report of work.
- Work within a multidisciplinary team (MDT) and report to the Health Team Manager; clinically supervised by the B7 Clinical Lead Nurse.
- Support and potentially clinically supervise a Band 5 Nurse or Nursing Assistant in the North LD team at Kelling Hospital.
- Develop working relationships with external stakeholders, including care providers and acute hospitals.
- Communicate with the Community Learning Disability Team and wider Integrated Care System to ensure cohesive care delivery.
- Work in partnership with people with lived experience, including carers and family; collaborate with other professionals across disciplines, including social care.
Qualifications
- NVQ 3, or equivalent knowledge and experience
- Foundation degree programme, or equivalent demonstrable knowledge and skills
- GCSEs Grade C or above in English and Maths, or equivalent;Experience of working in a health or social care setting
- Experience of working with people with learning disabilities
- Experience of working with people in the statutory/voluntary/private sector
Desirable criteria
- Experience of working in people\'s homes
- Experience of working with people at the end of life; knowledge of additional specialist areas through in-house training and short courses
