This particular role is based within the county of Hertfordshire. The Community Nurses will be expected to help and support the rest of the team with day-to-day duties and report to the Team Leader.

Requirements:

• Must have a degree in nursing and a minimum of 1 year's solid experience in Community Nursing.
• Responsible for managing a client caseload with support and guidance.
• Maintain up-to-date client records and ensure confidentiality.
• Act in accordance with NMC registration requirements and Code of Conduct.
• Provide professional nursing assessments supporting the commissioning process.
• Undertake clinical procedures appropriate to client care and maintain competency.
• Coordinate and evaluate care plans, working directly with some clients.
