Job description

We currently have a number of exciting opportunities for theappointment of dynamic, self-motivated Band 7 Patient flow TeamLeader to work as part of the Patient Flow Coordinator Team withinAcute Medicine based at St James University Hospital Leeds on a12 month secondment.

We are looking for enthusiastic individuals to take responsibility andactively manage patient flows across the Clinical Service Unit byimplementing and maintaining processes that safely expedite patientplacement and discharge.The jobholder is responsible for the timely and appropriate placementof patients requiring admission to the Trusts bed base and for theescalation procedure from the Emergency Department.

The role will give you an insight into the wider organization and theissues and drivers which shape current NHS services. You willdevelop advanced skills in identifying risks and solving complexissues, and gain valuable experience in balancing staff availabilityand skills against the needs of patients across the service.

Main duties of the job

You will be required:

To assist in the delivery of care standards and quality indicators tosupport safe and effective patient care.

To act as a clinical resource within the department and across theCare Group as necessary.

You should have knowledge in the field of hospital discharge and anunderstanding of the requirements of the national hospital dischargeand community support policy and operating model.To act as a positive role model.

The successful applicant will need to demonstrate recent changemanagement implementation, and a clear understanding of LeedsTeaching Hospitals Nursing & Midwifery Strategy.

Job responsibilities

The Jobholder will carry continuing responsibility for the effective andefficient management of the Patient Flow Team. They will provideprofessional, clinical, managerial leadership and supervision for thenursing team. The jobholder is responsible for developing andmaintaining the departments philosophy that has the patient at thecentre of compassionate safe and quality care delivery. The jobholderwill ensure cost effective use of resources.The jobholder will provide an overall responsibility for the timely flowof patients through the Specialty & Integrated Medicine CSU and willcontribute to the development of the speciality service as well ascontributing to the strategic direction of nursing within the Trust.

PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY

Leadership

Provide leadership to the clinical team

Support and inspire staff to provide efficient, effective, safe,quality care to patients

Promote the building of effective teams and collaborationbetween teams

Anticipate problems and take preventative or mitigatingactions, or have contingency plans available

Work with Matron to lead the development ofward/department nursing practice, reflecting the Clinical Service Unitand Trust business plans

Work with the Matron and or Head of Nursing to suggest anddevelop new roles/ways of working

Delegate and empower staff whilst maintaining overall, continuing responsibility and accountability

Line manage the nursing team, including individual and teamperformance, attendance, management and workforce planning

Ensure the effective deployment of staff is mapped againstthe flow of patients to provide safe and effective care

Support and participate in local and trustwide recruitmentevents

Manage the use of bank and agency

Investigate and respond to accidents, complaints, untowardincidents and other significant events

Deliver an effective, safe, quality service within budgetaryenvelope

Raise and respond to concerns with regards to risk, danger,malpractice or wrongdoing by following the LTHT whistle blowingpolicy and supporting processes

Act in accordance to the Nursing and Midwifery CouncilsCode: Professional Standards Practice and Behaviour

Fulfils the requirements as directed by the Nursing andMidwifery Council to maintain and revalidate registration

Supervise ward staff with regards clinical practice and patientexperience to promote the delivery of excellent clinical standards

Provide a visible clinical lead and role model to the nursingand wider team by delivering high quality compassionate care as anexpert practitioner

Ensure that documentation standards are maintained inaccordance with NMC and LTHT guidelines, whether written orcomputerised.

Promote and maintain positive working relationships betweenall staff, internal and external to the Trust, involved in the care ofpatients

Set, monitor and seek to continuously improve standards andthe quality of patient care, including those standards defined in localand national guidance

Maintain a safe working environment

Ensure the safe use and efficient management of equipmentand supplies

Promote compassionate evidence based practice, whereverpossible

Work with Matron to ensure that clinical governanceprocesses at ward/dept level encompass measures to ensure thequality of the fundamental and essential aspects of care

Balance clinical risk and clinical decision making againstcontext

Work with the Infection Prevention and Control Team andFacilities to monitor the environment to maintain and raise standards

Undertake duties on other wards or departments as andwhen required by the service

Act in such a way that safeguards the health and wellbeing ofchildren and vulnerable adults at all times. Is familiar with andadheres to the LTHT safeguarding policies.

Quality Improvement

Has a responsibility to improve the quality and safety of ourservices. It is expected that in fulfilling a clinical leadership role youwill be proactive in promoting and using the Leeds ImprovementMethod Contribute to quality improvement programmes as required
