Anexciting opportunity has arisen for a 12 month secondment to cover the TrustVTE Prevention Clinical Nurse specialist role.

Thepurpose of the venous thromboembolism (VTE) Prevention Clinical NurseSpecialist (CNS) role is to ensure high quality VTE prevention care isembedded throughout the organisation with the aim of reducing preventablepatient harm from hospital associated thrombosis. The role holder will be anintegral part of the multi-disciplinary thrombosis team and will takeresponsibility for leading on VTE prevention care, and ensuring adherence tothe NICE VTE guidance, to include; surveillance, analysis and improvement ofVTE risk assessment rates, hospital associated thrombosis cases, VTE staffeducation, VTE patient information and appropriate use of thromboprophylaxismeasures. Activity will involve data collection, analysis, presentation anddissemination, staff education activities, awareness initiatives and qualityimprovement work.

Asan integral part of the wider thrombosis nursing team, the post holder willparticipate in regular clinical activity in the nurse led anticoagulation andDVT services.

ClinicalResponsibilities

To develop first hand understandingand familiarity of the VTE prevention clinical processes used across theTrust.

To develop expert knowledge aroundVTE prevention and thrombosis management.

To act as a resource person for theTrust around VTE prevention and thrombosis management.

To work autonomously as an expertpractitioner in the nurse led anticoagulation and thrombosis servicesproviding safe and effective patient centred care.

To attend MDT meetings, ensuringall patients are represented.

To participate in theidentification and development of clinical protocols and strategies toenhance both the continuity and standard of specialist care whilst ensuringequity of access to the service.

Leadership

To accept responsibility for VTEprevention leadership locally and take initiative to seek performancesurveillance and data collection to inform robust quality improvement work.

To use initiative and build strongworking relationships to positively influence VTE prevention practice acrossthe Trust.

To actively support clinical staffto provide high quality VTE prevention care.

To contribute positively to theleadership of nursing within the care group acting as an effective rolemodel.

To actively involve service usersin providing feedback of their experience of the current service andsuggestions for improvement.

Job responsibilities

. PRINCIPAL DUTIES & AREAS OFRESPONSIBILITY

- Provide clinical leadership andacts as a role model to the specialist nursing team by delivering highquality care as an expert practitioner

Responsibilities for patient andclient care

- Assess, plan, implement andevaluates specialist care and treatments using advanced clinical knowledgeand clinical skills

- Plan specialist nursing serviceprovision organising and managing own specialist workload and that of thespecialist nursing team.

- Provides specialised advicecontributing to the diagnosis and care of patients.

Freedom to act

- Make clinical assessments andtreatment decisions within clearly defined policies and protocols

- Refer patients as appropriate toother members of the multi-disciplinary team

- Supervises and assesses theclinical care and outcomes of nurse specialists within own team.

Communication

- Maintains accurate clinical andother records using the Leeds Teaching Hospitals Trusts documentation

- Ensures appropriate clinical recordkeeping within the specialist nursing team.

- Support patients and theirfamilies, communicating sensitive or highly sensitive information, ensuringthey receive required information to enable them to participate in their caredelivery

- Liaise with Community Services (asappropriate) to ensure seamless provision of care delivery

Education and Training

- Provides and facilitates trainingand education of learners and other junior staff within own speciality area.

- Provides specialist training andeducation at post-graduate level within the University settings as agreedwith Matron.

- Actively participate in thedevelopment of own personal development plan and performance review. Ensure all staff within team have appraisaland personal development plan in line with Trust policy.

- Participate in specialist national,regional and local conferences as agreed with Matron.

- Provide education and training topatients and their carers.

- Keeps up to date with developments,advances within the specialty

Leadership and management

- Provide leadership to the clinicalteam working with Matron, develop new roles/ways of working

- Has responsibility for the linemanagement of the specialist nursing team including recruitment, performancemanagement and workforce planning

- Ensures that workload andperformance monitoring data for the team is collected, collated and utilisedto ensure the effective and efficient working of the team. Reviews thisinformation with Matron on a regular basis.

- Investigate and respond toaccidents, complaints, untoward incidents and other significant events

- Work with Matron to promote thedevelopment of nurses undertaking the CNO Ten Key Roles as consideredappropriate

- Work with Matron to lead thedevelopment of nursing practice, reflecting the CMT and Nursing Directoratebusiness plans

- Responsibility for ensuringsetting, monitoring and seeking to continuously improve standards and thequality of patient care including those standards defined in the NationalBenchmarks for Essential Care

Qualifications
