Job summary
East Suffolk and North Essex NHS Foundation Trust is seeking an experienced Nurse Practitioner to join the Urgent Community Response Service (UCRS). You will deliver urgent care in patients' homes and care homes, applying advanced clinical skills to assess, diagnose, prescribe, treat and manage a range of urgent needs.
The role emphasizes autonomy, rapid assessments within two hours, independent prescribing, and preventing hospital admissions, with collaboration with GPs and community teams.
Qualifications
- Registered Nurse Prescriber qualification/registration.
- Experience in urgent care or community-based roles is desirable.
- Ability to work autonomously with clinical leadership.
Responsibilities
- Undertake rapid response assessments within two hours of referral.
- Assess, diagnose and manage urgent/complex presentations in community settings.
- Independently prescribe appropriate treatments and interventions.
- Deliver short-term, intensive support to prevent hospital admission and facilitate recovery at home.
- Manage patients with frailty and multiple long-term conditions.
