As aPre-Operative Assessment Clinical NursePractitioner you will be responsible for seeing patients in Pre-operativeAssessment clinic to undertake investigations relevant to the patientsassessment, supporting your team at all levels and providing cross Countysupport to our Pre-operative assessment department. You will also liaiseclosely with the Anaesthetic team and the wider Surgical MDT.Your rolewill also include triaging patients for appropriate appointments and Nurse inCharge shifts.

Our Nurse Practitioner role is part of a wider Pre-Op team.You will be working with Band 3s, 5s, 6s and 7s as well as members of ourCountywide Booking team. You will be rostered a variety of shifts includingclinical work, triage and Nurse in Charge. Ideally you will have completed a healthassessment module to undertake heart and lung examination to support our morecomplex patients. You should hold phlebotomy and ECG skills to support the teamwhere needed.
The role itself is evolving to encompass changes within PreOp at a National level. This may include pre-screening, diabetes management andworking alongside the anaesthetic team. You will be willing to embrace changeand help shape practice within the department.

Job Purpose:
Toensure the maintenance of high quality patient focused care at all times,through effective leadership of staff and managed resources.

Asa named nurse / key worker for a defined group of patients, workingautonomously, to take responsibility for:
Theprovision of accurate pre-operative patient information.
Ensuringthat all necessary follow up and discharge preparations are in place prior toadmission.
Conductingholistic clinical assessments of all patients awaiting surgery, makingappropriate onward referrals in a timely fashion.
Providinghealth promotion, making every contact count.
Managingthe unit in the absence of the Band 6 Junior Sister and the Band 7.
Ensuringthat own skills are developed and shared with team as appropriate.
Ensuringeffective communication with patients, relatives, carers and themultidisciplinary team.
Participationin preceptorship programmes as appropriate and act as a facilitator / mentor /preceptor / role model to other members of the team.
Providingsupervision, support and education to unqualified staff, students and new staffteam members.
Liaisingwith Consultants and their teams in the event of surgical cancellations.
Workingalongside the Clinical Anaesthetic Lead and anaesthetic team.
KeyDuties:
PatientCare Responsibilities
Actingas a Named Nurse/Case manager to a cohort of patients from POA to admission.
Responsiblefor assessing, planning, implementing and evaluating programmes of care withinthe framework of team nursing.
Evaluationof patient health history and identifies previously unknown health concerns.
Ableto undertake advanced clinical health assessment including cardiac andrespiratory assessment and examination.
Independentlymake onward referrals to an additional service as appropriate e.g. GeneralPractitioner, Specialist Nurse.
Followup advice, referral, investigations for patients presenting with significant comorbidities.
Actsas a resource and provides advice on the management of patients to staff atBand 5.
Toco-ordinate the patients discharge arrangements in accordance with WAHTdischarge policy.
Topromote a patient-focused approach to care in collaboration with all relevanthealth professionals.
Toobtain an accurate and concise medical and medicines histories from the patientusing interpersonal skills to facilitate the assessment of the patient for bothminor and major surgery.
Usesinformation obtained during the assessment, to determine the patients fitnessto proceed for surgery, demonstrating the appropriate action to take in theevent of not fit to proceed.
Ensurethe patient has an understanding of their proposed operation by providingaccurate peri-operative information both verbally and in writing.
Provideinformation that will enable the patient to make choices about adopting ahealthier lifestyle and makes a referral, as appropriate, for optimisation oftheir health before surgery.
Arrangesand co-ordinates any assessment and/or investigations needed near the time ofsurgery, in accordance with local and national guidance.
Identifiesabnormal readings and acts on the findings in an appropriate and timely manner.
Toassist in establishing and monitoring of protocols and care pathways.
Toensure effective communication with patients, relatives and other staff.
Tosupport peoples equality, diversity and rights at all times.
Tosafe guard vulnerable adults in accordance with agreed policies and procedures.
